At Jefferson Center, we are committed to providing inclusive and equitable care to individuals from diverse backgrounds. As a Healthcare Coordinator, you will play a vital role in linking adults, children, and their families to comprehensive medical, behavioral health, and psychosocial services.
Key Responsibilities- Assess barriers to health and determine appropriate service referrals.
- Research and prepare materials to provide education and support to clients.
- Collaborate with medical providers to provide brief-focused screening and intervention through a collaborative care model.
- Facilitate weekly psychiatric consultation support between medical providers and established psychiatric consultants.
- Coordinate the delivery of services to patients, including collaborating with clinical and other staff to address patients' needs.
- Facilitate information exchange and process continuity of care documents for clients.
- Connect patients to primary and specialty care resources within the community to address identified health concerns.
- Participate in case review sessions and provide timely documentation and required reports for all care coordination activities.
- Bachelor's Degree in a related field is required; a Master's degree in Social Work, Psychology, Public Health, or a related field is preferred.
- 2+ years of experience in case management or healthcare navigation in a medical care setting is required.
- Proficient skills in care coordination, healthcare navigation, charting, documentation, collaboration, and advocacy are required.
