Position Overview: We are looking for a dedicated Nurse Care Manager to enhance the quality of life for seniors who are homebound. This position is centered on overseeing Chronic Care responsibilities for both patients and healthcare providers. The Nurse Care Manager will collaborate closely with the clinical team, healthcare providers, intake personnel, and community care partners, playing a vital role in the coordination of patient care.

Key Responsibilities:

  • Revise and update patient care plans
  • Prepare patients and healthcare providers for appointments
  • Facilitate care coordination with community partners
  • Oversee post-hospitalization care plans
  • Work in collaboration with the Interdisciplinary Care Team (IDC)

Essential Skills and Qualifications:

  • Expertise in EMR software
  • Strong analytical and problem-solving capabilities
  • Exceptional customer service skills
  • Clinical experience
  • Desire to further develop clinical knowledge

Minimum Qualifications:

  • Valid Registered Nurse (RN) license
  • A minimum of 3 years of clinical experience
