Position Overview: We are in search of a dedicated Nurse Care Manager to enhance the well-being of homebound elderly patients by overseeing Chronic Care responsibilities for both patients and healthcare providers. This position involves close collaboration with the clinical team, healthcare providers, intake personnel, and community care partners, making the Nurse Care Manager essential in the coordination of patient care.
Key Responsibilities:
- Revise and update patient care plans
- Prepare patients and healthcare providers for consultations
- Facilitate care coordination with community partners
- Oversee post-hospitalization care plans
- Engage with the Interdisciplinary Care Team (IDC)
Essential Skills and Qualifications:
- Expertise in EMR software
- Strong analytical and problem-solving capabilities
- Exceptional customer service skills
- Clinical experience
- Desire to further develop clinical knowledge
Minimum Qualifications:
- Valid Registered Nurse (RN) license
- A minimum of 3 years of clinical experience
