Job Details
Our Palliative Care Coordinator ( RN ) will:
Collaborate with the Palliative Care Team in planning and implementing new methods of delivering nursing care; Coordinate and complete palliative care consults Incorporate ethical principles and professional standards in the care of patients and families experiencing life-limiting, progressive illnesses as well as identifying and advocating for the wishes and preferences of parents and families. Promote the provision of comfort care to the dying as an integral component of nursing care. Act as a consultant to patients, families, and staff (hospital and hospice) regarding end of life issues and palliative care Assist with education and training of hospital employees; MDs, nurses, medical students, residents, social work and other members of the healthcare team as appropriate. Assist with education of the community regarding palliative care and associated services Coordinate along with the Administrative Director and Medical Director, annual strategic planning for the palliative care program Maintain oversight of the program budget, review of performance measures, and use of staff resources proportional to activity and patient care Assure compliance with the Joint Commission and other regulatory standards Participate in quality assurance and assure compliance with and implementation of any resulting recommendations Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RNs), and ancillary staff to optimize patient outcomes and satisfaction. Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education.What you’ll bring:
Passion for Palliative Care Nursing 3+ Years of working RN experience Prior working experience with home care, hospice, palliative care, advanced directives, pain management, symptoms control and discharge planning Education: Graduate of an accredited School of Nursing requiredBachelor of Science in Nursing (BSN) degree required Master’s degree preferred Licensure: New York Registered Nurse (RN) License & Registration Certification: Basic Life Support (BLS) certification requiredSalary Range
USD - USD /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
