ORGANIZATION BACKGROUND

Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County. Our organization follows a hybrid work structure where employees work both remotely and from the office, as needed.

PROJECT BACKGROUND

The Department of Health Services (DHS) Housing for Health (HFH) program is a cohesive team of clinicians, community health workers, program administrators, and other professionals who collaborate with multiple County departments, community-based service providers, cities, and other partners to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. HFH works to meet the needs of people with complex health and behavioral health conditions. In response to the homelessness crisis in Los Angeles County, the Los Angeles County Departments of Health Services (DHS), Mental Health (DMH), and Public Health (DPH) are coming together to form regional, field-based, multidisciplinary teams called Interim Housing Assessment and Action Teams (IHAAT) to address gaps in behavioral health and physical health services among functionally disabled People Experiencing Homelessness (PEH) who are living in interim housing (IH) and connect them to stabilizing services (such as in home caregiving, higher level of care, or enhanced care management/community support). The IHAAT teams will comprise of both medical and behavioral health staff and will prioritize and expedite social and housing navigation services in all eight (8) Los Angeles County Service Planning Areas (SPAs). The DHS teams will be funded through a 5-year grant from the state-sponsored Housing Homeless Incentive Program (HHIP) and administered through the two largest Medicaid managed care plans (MCPs) in LA County: LA Care and Health Net.


As part of the initiative for the Interim Housing Assessment and Action Teams (IHAAT), also known as the Interim Housing Outreach Program (IHOP), Housing for Health will sponsor the Activities of Daily Living (ADL) program to increase access and support for PEH with ADL needs in interim housing (IH). These teams will perform comprehensive assessments of high-risk PEH in IH settings, refer them to supportive services, increase their function and safety within their current environments, and assist them with transitioning them to higher levels of care, as appropriate. The Interim Housing Outreach Program (IHOP) teams will serve clients in shelters, motels, and tiny homes located in all eight Service Panning Areas (SPAs) in Los Angeles County. DHS-sponsored IHOP staff, hired by the DHS Housing for Health (HFH) program, will include advanced practice providers, nurses, emergency medical technicians, and occupational therapists.

The Staff Assistant, IHOP will assist and support all IHOP operations. 

RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

Assist both clinical director, clinical management team, as well as program manager of IHOP operations. Serve as resources for IHOP staff (clinicians and admin team) during onboarding and day-to-day operations, including helping them with equipment malfunctions, scheduling questions, supplies needed, ordering. Support field-based scheduling of clinical teams and supporting clinical management of systems used for scheduling. Works directly with HFH management staff to fulfill an array of administrative task that include data entry and tracking, client follow up, and operations within the HFH database, Comprehensive Health and Accompaniment Management Platform (CHAMP), HMIS (Homeless Management Information System). Processing, editing, and creating/developing an array of various documents (e.g., Word, Excel documents, etc.). Frequent use of an array of Microsoft Office Suite software programs such as Word, Excel, PowerPoint, Access, Adobe Reader, One Note, Outlook, Publisher, Teams, Outlook Screens in-person inquiries and telephone calls; furnishes requested information, refers inquiries, calls, and emails to others better qualified to assist, and personally takes care of inquiries and calls which do not require the attention of HFH Makes appointments and arranges conferences, trainings, and meetings for HFH personnel as applicable; coordinates and schedules applicable internal and external personnel (e.g., HFH personnel and/or HFH community/contractor agencies) with respect to HFH trainings, meetings, events; maintains training calendars, vacation calendar; preparing agenda, sending communication, and taking meeting notes with f/u for deliverables. Prepares and photocopies meeting materials, agendas, handouts, training materials, presentations, etc.; prepares and distributes internal and external notices, bulletins, letters, and memoranda as applicable; develops and maintains office files specific to supply requests, time off requests, personnel files, trainings, Attends meetings, as needed with HFH personnel, takes notes, prepares meeting minutes, action items, May prepare and/or assist in the preparation of reports, spreadsheets, documents, literature, material, forms, etc.; may search records and files for data in which a required and/or sampling of information, data, is needed in selecting or abstracting material, documents, client information, etc. May assist in the collection, analyzation, and or interpretation of data as needed May have inadvertent access to confidential HFH client data Performs other clerical, general administrative/technical assistance duties as needed by Clinical Director and Program Manager

DESIRABLE QUALIFICATIONS

Two years of experience in either (1) a staff capacity analyzing and making recommendations for the solution of problems of organization, procedure, program, budget or personnel, or (2) a highly responsible secretarial capacity, assistant, or coordinator or (3) a responsible supervisory clerical capacity.

Excellent written and verbal communication skills, including the ability to express technical concepts clearly to both technical and non-technical audiences. Ideal candidates will be organized, diligent, and have strong MS Excel and data processing skills. Ability to make thoughtful decisions and exercise sound judgement; be self-directed, resourceful, and creative in problem solving Existing relationships and trust within communities of focus Ability to work as part of a team in a constantly changing environment Feel comfortable in environments such as homeless shelters Strong project management and relationship building skills. Effective interpersonal and organizational skills Ability to work effectively in a team-based setting Ability to learn quickly and adapt to shifting priorities. Comfort working with data systems and learning new data systems. Strong listening, negotiation and conflict resolution skills. Knowledge of LA County programs that are designed to serve the underserved. Provide empathetic care with a harm reduction, “whatever it takes” philosophy Skill in organizing new hires onboarding; scheduling of interviews, notetaking during interviews, communications to internal and external HR department for onboarding communication. SCHEDULING

Specific schedule to be determined up to 40 hours per week. Proposed field-based teams operating hours are Monday through Friday from 7:00 a.m. to 6:30 p.m.; set scheduled will be addressed at time of onboarding but ideally 8am – 5pm with some possibility for variation based on field-based teams’ hours. Hybrid model consisting of office-based work, remote work, and minimal field work. Be available to travel throughout LA County’s Service Plan Areas 1-8, if necessary.
