 If you are located within the New Jersey territory and willing to travel up to 50% of your time to assigned territory, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

Make outbound calls to assess members' current health statusDemonstrate an ability to deliver effective telephonic case management in a manner that promotes behavior change for the NJIdentify gaps or barriers in treatment plansProvide patient education to assist with self-managementInteract with Medical Directors on challenging cases including presenting at roundsCoordinate care for membersMake referrals to outside sourcesCoordinate services as needed (home health, DME, etc.)Educate members on disease processesEncourage members to make healthy lifestyle changesDocument and track findingsMake “welcome home” calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:

Current, unrestricted RN license in New Jersey2+ years of RN experience in a hospital setting, acute care, direct care experience and/or experience as a Case Manager1+ years of paid professional experience working directly with the elderly or physically disabled in an institutional or community setting1+ years of experience in long-term care, home health, hospice, public health or assisted livingAbility to navigate a Windows environment, utilize Outlook, and the ability to create, edit, save and send documents utilizing Microsoft Word and ExcelDemonstrated ability to talk and type proficiently at the same timeAccess to reliable transportation and the ability to travel up to 50% within assigned territory to meet with employees, members, and providers

Preferred Qualifications:

Bachelor's degree or greaterCertified Case Manager (CCM)DSNPExperience working with populations with special needsExperience/exposure with discharge planningExperience in utilization review, concurrent review and/or risk managementExperience with government health programs (Medicaid / Medicare)Case Management experienceBackground in managed careBilingual - English/SpanishProven solid organizational skills and multitasking abilities 

New Jersey Residents Only: The hourly range for this role is $28.03 to $54.95 per hour. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and k contribution (all benefits are subject to eligibility requirements).
