St. Luke’s Health System in Boise is seeking a part-time Care Coordinator to join our Inpatient Pediatrics team
What You Can Expect:
As a patient care coordinator, you will work collaboratively with the entire care team – both inpatient and outpatient - to facilitate safe transition from the hospital setting to home for pediatric patients and families. We coordinate a variety of discharge needs ranging from equipment delivery, educating patients and/or family members, completing medication prior authorizations and collaborating with home health agencies in the community to name just a few. Though we are not doing bedside nursing, we are very active members of the patient care team by participating in family centered rounds, facilitating care conferences, and advocating for our patients and families. Skills that lead to success in this role are detail oriented, enjoy fast paced work environment, creative problem solving and ensuring family centered
and comprehensive care of our littlest patients.
Schedule:
Monday - Friday
Shift: 8 hour shifts | 8 am - 5 pm (variable)
Includes every fourth weekend and rotating holidays
Qualifications:
Education: Bachelors’ of Nursing Degree Experience: 2 years of RN experience Licenses/Certifications: Current RN License in the State of Practice AND American Heart Association (AHA) BLS Provider certification
