Job Summary

The Support Coordinator/Service Coordinator/Case Manager provides case management activities necessary to meet the needs of clients assigned to the consolidated care team.

This role is responsible for developing individualized care management plans, implementing care plans through brokering and coordinating services, and monitoring and evaluating clinical outcomes to ensure services are provided in a professional, comprehensive, and cost-effective manner.

Key Responsibilities

  • Complete assessments to identify client needs and desires for service delivery.
  • Develop and modify Person-Centered Service Plans annually or as needed.
  • Ensure client choice of providers by providing information for informed decision-making.
  • Coordinate with other individuals and entities essential in care delivery, including other care coordinators.
  • Research and develop alternative solutions to client needs.
  • Ensure confidentiality regarding sensitive client information.
  • Attend required trainings and recertification classes.

Qualifications

Minimum of a bachelor's degree in a human services field from a nationally accredited university or college and 12 months supervised work experience with the population being served.

Experience with Microsoft Office and state-maintained databases.

Maintain proficiency in company-sponsored training and certifications.

CPR/First Aid certification and updated state-related clearances.

Valid Driver's License with personal vehicle insurance coverage and current vehicle registration.
