Laurels of Green Tree Ridge is seeking a motivated assistant to fill our social services assistant position. The assistant will support social services in meeting resident social and/or emotional needs that affect the residents' ability to achieve their highest practicable psychological well being. They will participate in the development of residents' comprehensive care plans, follow policies and procedures to provide social services to residents in compliance with federal, state, and local regulations.

Responsibilities:

  • Assists Social Service Director with scheduling appointments, taking notes and transcribing information into resident's medical record
  • Assists Social Service department with admissions, discharges and maintains communication with resident and families.
  • Assists guests with financial and legal matters as delegated to by the Social Service Director (e.g., applying for pensions, referrals to lawyers, referrals to funeral homes for preplanning arrangements);
  • Provides discharge planning services (e.g., helping to place a guest on a waiting list for community congregate living, arranging intake for home care services, assisting with transfer arrangements to other facilities, working with outside agencies toward safe discharges);
  • Provides or arranges needed counseling services and psychiatric medication management as indicated.
  • Assures documentation is accurate and timely in MDS care plans.

Qualifications:

    • High School Diploma
    • One year of experience in a healthcare setting is preferred.
    • One year of experience as an administrative assistant is preferred.
