Assist patients and health care needs as directed by clinician during Emergency Department visit with a focus on Substance Abuse DisorderLocations- Cleveland Medical Center (Main Campus), UH Elyria, UH Lake West/Tripoint Travel within community and home visitsCollaborate with healthcare providers in assisting patients with education and support to improve long term healthcare outcomes.Provide intensive case management to patients and assist with access to substance use and medical care, barriers in obtaining necessary resources; both community and insurance based.Initiate and lead unbiased and trauma-informed conversations with any and all patients regarding their personal history of substance use.Provide compassionate and professional conversation and care throughout complex patient conditions including during episodes of substance use.Act as a guide in navigating transitions of care during a patient's health care journey.Providing psychosocial assessment/intervention to assist patients/families (including clinically complex) in addressing barriers to receiving appropriate healthcare.Monitoring and updating a comprehensive, complex care plan for individual patients based on identified psychosocial needs, discharge needs and overall population data.Maintaining a current working knowledge of UH network and substance use services and promoting utilization of such services.Effectively communicating and collaborating with the complex care team, and, where appropriate, patient's care providers, population health analysts, and community organizations using a multidisciplinary approach focused on improving the patients overall health and wellness.including performing 12 lead EKGs, blood draws, initiate Hep-locks, bedside point of care testing, aerosol treatments. Ensures the daily completion of point of care testing quality controls, defibrillator checks, refrigerator checks, transports. Performs ABGs and gain IV access as directed by order. Prepares patients and care environment for exam, specimen collection and anticipates needs of patients and providers proactively. Performs delegated basic procedures as described by their annual competency based tool and orientation program.Requires: Active State of Ohio Paramedic certification.Responsible and accountable for safe clinical practice, focused on expanding knowledge and skills.Consistently provides effective relationship-based care as part of the interdisciplinary team to a variety of complex patients.Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role and seeks mentoring in the process.15% travel within community and home visits.Qualifications:Graduated from a state accredited paramedic program. State of Ohio Paramedic Certification requiredPrior experience in emergency/hospital care or related field preferredExperience QualificationsField experience with focus on complex substance use disorder patients in hospital and home health environment preferred. Preferred andExperience working with Electronic Medical Record application and other computer based patient management tools preferred. PreferredSkills and AbilitiesNaloxone administration training. (Required within 90 days of hire)Knowledge in navigation of the healthcare system, including benefit coverage and health treatment protocols. (Preferred proficiency)Excellent verbal and written communication skills. (Required proficiency)Proven time management and organizational skills.
