You will work closely with APRNs, nurses, social workers, therapists, and other specialists to develop personalized care plans that meet the individual needs of each participant and will provide and coordinate all the healthcare and support services individuals need to remain living as independently as possible at home..In addition to providing in-home care and support, you will have the opportunity to work in our state-of-the-art facility, equipped with the latest medical technology. This will allow you to deliver high-quality care in a comfortable and convenient setting.We are looking for a physician who is not only highly skilled and knowledgeable but also compassionate and empathetic. Our participants deserve the best care possible, and we are committed to finding a physician who shares our values and dedication to excellence.Requirements:•Education: Medical Doctor with emphasis in geriatrics. Board certified in Internal Medicine, Family Practice, or Geriatric practice desired. Current state license and DEA registration required. Meets requirements of 42 CFR Part 410.20.•Experience: A minimum of one year experience working with a frail or elderly population required.•Board certified in Internal Medicine, Family Practice or Geriatrics.•Florida state license and DEA registration.•Cardio Pulmonary Resuscitation (CPR) certification required.•Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.•Must have a valid driver's license, proof of insurance and have means of transportationEmpath Health values diversity as it strengthens our community and care.
