Responsibilities:
In your job, you will have the ability to make a difference in a patient’s journey by:

 

Work under direct supervision and in tandem with the Oncology Nurse Navigator to coordinate patient care through all phases of the care continuum.

Support the Oncology Nurse Navigator in reporting patient and family challenges and assist with potential solutions, while considering the patient’s values and cultural, spiritual, and social needs.

Coordinate patient needs for appointments and schedule appropriately to expedite the plan of care.

Ensure closure of all referral loops, internal and external, through procurement and inclusion of referral documentation in the medical record in a timely manner.

Appropriately document all patient interactions and services provided within the electronic medical record.

Act as a liaison and advocate for patients, families and caregivers when assisting the multidisciplinary team.

Serve as a liaison/contact for referring physicians, external medical offices, and patients in a timely, professional manner.

Participate in continuous process improvement of patient outcomes, assuring appropriate utilization of resources to increase patient, family, and caregiver satisfaction.

May schedule outpatient appointments/testing/surgeries and hospital admissions upon request.

Obtain necessary pre-certification as required.

May communicate to patient about surgeries or tests being scheduled and potential prep work needed, inquiring about test specifics from necessary parties and gathering patient information as needed.

May be responsible for follow up on all pre-operative tests to ensure cleared for surgical procedure and communicates with physician and patients if further testing is required before surgery is performed.

Where applicable, basic understanding of medical terminology for tests and surgical procedures, including coding and ability to look up codes specific to diagnosis, surgery, and scheduling procedure.

Qualifications:
Minimum Qualifications:

 

High School Diploma or equivalent; advanced education preferred.

Minimum four (4) years of experience working in a clinical health care environment.

Oncology-related education, planning, management, or similar experience is required.

 

What You Bring to the Team:

 

Medical Assistant (certified) experience with medical, radiation, and /or surgical oncology/hematology patient population preferred.

Strong organizational and interpersonal skills.

Strong knowledge and proficient use of an electronic medical record (EMR) system.

Proficiency in reviewing medical records and other patient care information.

Strong knowledge and understanding of HIPAA compliance and patient confidentiality regulations and policies.

Ability to develop collaborative relationships and work in teams, including medical, surgical, and radiation oncologists and referring providers.

Strong written and verbal communication skills, including medical terminology and therapeutic interventions available.

Strong customer service skills.

Knowledge of local community resources.

 

The safety of our patients and staff is our top priority. Masks may be requested in all Arizona Oncology facilities regardless of vaccination status. COVID vaccination is recommended.
