Case Manager

As a Case Manager, you are responsible for providing assistance to the Veterans (Vet) in need at the SEVSC. Case Managers seek out program eligible veterans, ascertains their needs for services and informs them of the Vet Center programs. Incumbent will interview veterans to determine specific problems and conducts intake and needs assessments, identifies symptoms leading to clinical diagnosis and develops comprehensive treatment plans for readjustment. Serves as case manager and provides clinical case consultation and assistance to counselors on complex cases; provides crisis intervention, stabilization and follow-up to veterans. Crisis intervention requires the incumbent to work independently in uncontrollable environments where established procedure and normal practices may not apply.
Other duties include:
Meets with SRO permanent housing clients twice monthly and Transitional housing participants clients weekly.
Generate correspondence and reports and other statistical information to be sent to outside community based organizations, government agencies, and collaborative partners in the community.
Prepares forms and charts for tracking client movement within the SEVSC
Prepare end of the year program reports Executive Director
Develops appropriate treatment plans using available resources to include initial assessment/clinic input, and makes continuing decisions regarding client treatment and referral.
Independently concludes the appropriate action, even in the event that actions cause serious impact on the life of the client served.
Provides referral services to other VA and Community resources.
Establishes a continuing relationship with the client, evaluating progress towards goals and adjusts treatment plan if necessary.
