Join Community of Hope as a Housing Stability Case Manager in our Homelessness Prevention Program and play a vital role in keeping families safely housed. You'll provide personalized support, connect clients to essential community resources, and empower them to overcome barriers to stability. This position is located at Conway Health and Resource Center in Southwest, DC.

What You'll Do
- Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support.
- Meets weekly and as needed with families on caseload to develop case plans, review progress, and to follow-up on tasks and services.
- Coordinates with available employment supports to ensure that clients' employment and educational needs are being effectively addressed.
- Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate, including to COH Therapist and other community resources, as appropriate.
- Completes required assessments with families.
- Maintains regular communication in accordance with program contract guidelines with all families on caseload and documents progressive engagement efforts with difficult to engage families.
- Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs.
Requirements:
Must-Haves
- Bachelor's degree in social work or related field required or 4 years social service experience.
- Minimum one year work experience in social service or related field.
- Strong interpersonal, conflict-resolution, and organizational skills.
- Ability to work independently and as a team.
- A valid driver's license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
Nice-to-Haves
- Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly.
