Overview

This position requires a compassionate individual with strong communication skills and the ability to work collaboratively with interdisciplinary teams. Proficiency in Microsoft Word and Excel is essential, as is the capacity to thrive in a high-paced environment. The Social Services Assistant plays a crucial role in supporting residents' well-being and advocating for their needs throughout their stay.

Purpose of the Position

To assist in planning, developing, implementing, and evaluating social service programs that meet the medically related emotional and social needs of residents, in accordance with federal, state, and local regulations.

Key ResponsibilitiesAdmissions & Orientation

· Assist with the admission process, including interviews with residents and families.

· Complete social histories and psychosocial assessments.

· Orient new residents to the facility and help them adjust to the environment.

Care Planning

· Participate in interdisciplinary care plan meetings.

· Develop and implement psychosocial care plans.

· Encourage resident and family involvement in care planning.

Resident Advocacy & Support

· Address resident and family concerns through counseling and problem-solving.

· Assist with room changes and transitions within the facility.

· Promote and protect resident rights and dignity.

· Assist Patient / Residents with Admission Agreement

Discharge Planning

· Coordinate discharge plans and assist with arranging transportation and follow-up services.

· Ensure continuity of care post-discharge.

Documentation

· Maintain accurate records of social service interactions and assessments.

· Document care plans, progress notes, and resident preferences.

Advance Care Planning

· Discuss and document advance directives and end-of-life wishes.

· Ensure staff are informed of residents' code status and preferences.

Staff Collaboration & Education

· Work with nursing, medical, and administrative staff to support resident needs.

· Educate staff on psychosocial issues and resident rights.

QualificationsEducation

· Minimum: High school diploma or equivalent.

· Preferred: Bachelor's degree in social work, Psychology, Sociology, or related field.

Experience

· Experience working with elderly or disabled populations.

· Familiarity with institutional care settings and geriatric issues.

Skills

· Strong communication and interpersonal skills.

· Ability to document accurately and use electronic health records.

· Knowledge of medical terminology and social service regulations.

Job Type: Full-time

Pay: $40, $56,000.00 per year
