Job Summary

The Virtual Care Coordinator is a critical role responsible for managing the seamless intake process for new patients referred to our organization. This individual serves as the primary point of contact, ensuring patients move efficiently from initial preliminary contact through referral verification, clinical coordination, and final scheduling. Success in this role requires exceptional organizational skills, detailed knowledge of healthcare referral processes, and a commitment to outstanding patient service.

Key Responsibilities

Patient Intake and Communication

  • Conduct professional, preliminary phone calls with new potential patients to gather accurate demographic, insurance, and preliminary clinical information.
  • Clearly communicate intake processes, required documentation, and organizational policies to potential patients.
  • Serve as the primary liaison between patients, referring physicians/clinics, and internal clinical staff to facilitate a smooth transfer of care.

Referral and Coordination Management

  • Accurately process, track, and manage all incoming physician referrals, ensuring compliance with established protocols.
  • Proactively work with referring clinics to acquire necessary medical documentation (e.g., onboarding paperwork, referral forms) needed for clinical review.
  • Coordinate with the billing team on confirming patient insurance eligibility and benefits for the requested services and assist in securing necessary prior authorizations as required before scheduling.
  • Manage referring provider notifications and updates throughout patients' tenures in the rehab program.

Scheduling and Workflow

  • Manage the scheduling process for initial patient appointments based on patient preference and provider availability.
  • Utilize the Electronic Health Record (EHR) and scheduling systems to document all communication and maintain up-to-date patient appointment records.
  • Collect patient copays in the Electronic Health Record (EHR) prior to appointments when necessary.

Compliance and Documentation

  • Ensure all patient interactions and referral activities strictly adhere to HIPAA regulations and organizational privacy standards.
  • Maintain meticulous and timely documentation of all steps in the referral lifecycle, from initial call to scheduled appointment.
  • Identify and flag complex referral issues, non-compliant documentation, or potential patient barriers to the Director of Patient Experience for resolution.

Qualifications

Education and Experience

  • Associate's Degree in healthcare administration, business or related field, or certified clinical medical assistant or similar experience required
  • Minimum of two (2) years of experience in a medical office, hospital, or high-volume healthcare setting, with a focus on scheduling, patient registration, or referral management.
  • Demonstrated knowledge of medical terminology, insurance verification, and authorization processes.

Skills and Competencies

  • Technical Proficiency: Advanced ability to navigate and utilize Electronic Health Record (EHR) systems (e.g. eClinicalWorks) and standard office software (Google Workspace).
  • Communication: Exceptional verbal and written communication skills with the ability to convey complex information clearly and professionally to patients and clinical partners.
  • Organizational: Strong organizational, time management, and multitasking skills with proven ability to manage high-volume workflows under pressure.
  • Attention to Detail: Meticulous approach to data entry and verification, ensuring accuracy in scheduling and documentation.
  • Customer Service: A compassionate and professional demeanor focused on delivering a positive patient experience.
