About Us:
We are a growing Home Care Agency focused on providing quality care to our clients. We are seeking a Skilled and Non Skilled Care Coordinator to join our team and support caregivers, clients, and field operations. Bilingual Spanish is a strong plus.
Position Overview:
The Care Coordinator will handle daily scheduling, respond to client and caregiver needs, and help ensure smooth operations across both skilled and non skilled services.
Responsibilities:
- Coordinate schedules and assignments for HHAs, PCAs, and skilled nursing staff
- Answer incoming calls and assist clients, caregivers, and nurses
- Enter and update client and caregiver information in the system
- Support the nursing team with admissions and supervisory visits
- Handle schedule changes and emergencies professionally
- Maintain compliance with agency policies and state regulations
- Assist recruitment and onboarding teams when needed
- Provide strong customer service to clients and families
Requirements:
- Prior home care coordination or scheduling experience preferred
- Strong organizational and multi tasking skills
- Excellent communication and customer service skills
- Bilingual Spanish English is a plus
- Ability to work in a fast paced environment
- Experience with HHAExchange, ClearCare, or similar systems is helpful
