PCA Coordinator (Bilingual Spanish/English)

Role Summary
As a PCA Coordinator, you will be responsible for coordinating Personal Care Aide services by managing schedules, supporting aides, and maintaining ongoing communication with clients, families, referral sources, and internal teams. You will ensure that care plans are executed accurately and that clients receive consistent, compassionate, and reliable care.
Key Responsibilities
Client & Care Coordination
- Coordinate PCA services to ensure appropriate coverage and continuity of care
- Maintain ongoing communication with clients, families, referral sources, and internal departments
- Serve as the primary point of contact for client inquiries, concerns, and service updates
Scheduling & Staffing
- Schedule and manage PCA assignments to meet client needs
- Address call-outs, schedule changes, and urgent coverage needs
- Support and guide PCAs to ensure adherence to care plans and agency standards
Systems & Documentation
- Maintain accurate documentation and updates in HHAeXchange
- Ensure compliance with agency policies and NYS DOH requirements
- Collaborate with intake and clinical teams to support smooth service delivery
Requirements
- Bilingual: Fluent in Spanish and English (required)
- Minimum 3 years of experience with a Licensed Home Care Services Agency (LHCSA)
- Prior experience in PCA coordination, HHA coordination, staffing, or scheduling
- Working knowledge of HHAeXchange
- Strong communication, organization, and problem-solving skills
- Ability to build trust with PCAs and foster strong working relationships
- Highly motivated and comfortable in a fast-paced, high-volume environment
