As a Clinical Director, you will play a key role in supervising and training our primary care providers (PCPs) while maximizing clinical quality and patient outcomes.

Key Responsibilities:

  • Provide comprehensive care for patients with both acute and chronic illnesses, especially among older adults.
  • Take full responsibility for patient care and outcomes, consulting specialists as necessary while maintaining involvement in detailed patient care.
  • Engage with hospitalists when your patients are hospitalized, ensuring continuity of care.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
  • Lead a care team, which includes a medical assistant, care facilitator, and care coordinator, for patients visiting the office.
  • Coordinate care for patients who may be unable to come to the office, including those in hospitals, skilled nursing facilities, or homebound settings.
  • Lead weekly meetings, including Super Huddles and Transforming Care Meetings, to ensure high standards of care.
  • Actively participate in center management, including coverage for absent providers and contributing to patient recruitment efforts.
  • Mentor and coach PCPs to achieve outstanding clinical outcomes.
  • Partner with operations management to enhance financial performance and increase center membership.
  • Perform additional duties as assigned to support our commitment to quality healthcare.

Knowledge, Skills, and Abilities:

  • Proficiency in Microsoft Office Suite and familiarity with various applications used within the organization.
  • Willingness to travel locally and occasionally for training and recruitment purposes.
  • Fluent in English with strong verbal and written communication skills.
  • Ability to exercise independent judgment in all aspects of patient care.

Qualifications:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
  • Active MD licensure in your state of employment.
  • A minimum of 1 year of clinical experience is preferred, with experience as a ChenMed PCP being ideal.
  • Board certification in a relevant specialty is preferred; Board eligibility is required.
  • Must possess a current DEA number for controlled substances.
  • Basic Life Support (BLS) certification required within first 90 days of employment.

Competitive Salary:

$231,876 - $331,251
