This hybrid position focuses on providing high-quality clinical and psychosocial services to diverse patient populations, emphasizing coordinated, patient-centered care in a collaborative healthcare environment. Key ResponsibilitiesConduct comprehensive clinical and psychosocial assessmentsDevelop, implement, and manage individualized treatment and care plansProvide counseling, crisis intervention, and ongoing clinical supportMonitor client progress and update care plans as neededCollaborate with physicians, nurses, behavioral health providers, and interdisciplinary teamsAssist patients and families with access to community resources and social servicesSupport care coordination for patients with Medicaid and Medicare coverageMaintain timely, accurate, and compliant clinical documentationCoordinate with community partners to ensure continuity of care Qualifications & RequirementsLCSW / LMSW Candidates:Master’s degree in Social WorkActive LCSW / LMSW license in New York StateMinimum 1 year of professional experienceBilingual English/Spanish requiredStrong clinical assessment and communication skillsComfortable working in a hybrid care environmentAbout LanceSoftEstablished in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
