Job DescriptionJob DescriptionCare Coordinator

HealthCare Support is actively seeking a Care Coordinator to fill an opening with a Managed Care Organization in Temple Texas.

Daily Responsibilities
  • Will have 3 days of visits and 2 days of documentation weekly
  • Will have a caseload and work assigned tasks
  • Will complete H2060 assessment
  • Fill out and put required forms for each assessment
  • SC will make follow up calls
Required Qualifications:
  • Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field required
  • 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions (adult/geriatric not peds)
  • Microsoft Office, excel
  • Clear and active driver’s license, reliable transportation, and insurance

Pay Details: $25 - $26 per hour ($51 - $59k annually)
