Job Summary

The Registered Nurse is a key member of our team, responsible for planning, organizing, and directing home care services. With a focus on community health education and experience, this role requires a professional RN to build on the resources of the community to meet the needs of individuals and families within their homes and communities.

Responsibilities
  • Plan and direct home care services to meet the needs of individuals and families
  • Build on the resources of the community to provide comprehensive care
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and families on health management and self-care
Requirements
  • Registered Nurse (RN) with current and unencumbered license to practice professional nursing in the state of employment
  • Bachelor's degree in Nursing from an accredited program, preferred
  • One (1) to two (2) years of recent acute care experience in an institutional setting
  • One (1) year of home health care experience, preferred
  • Possess and maintain valid CPR certification
