The role

Position: Individualized Support Services Assistant Manager
Location: Wakefield, MA
Pay rate: $24.50 per hour
Hours: 40 hours per week, days and hours flexible depending on needs of Individuals and Program

Summary & Responsibilities

The Individualized Support Services Assistant Manager is a leadership role that includes individual records management, coordination of required weekly reports and documentation, and providing/coordinating daily living support to adults with developmental disabilities to help them live as independently as possible, using the home and community as the classroom. Manage the 118 Nahant St and the individuals residing there. 
  • Ensure that all required paperwork (ISP documentation, progress notes, etc.) and training is appropriately completed and submitted in a timely manner.  
  • Assist mentors to prioritize areas of emphasis for everyone. Areas include, but are not limited to, medical, social, spiritual, cultural, recreational, nutritional, financial, and educational support.
  • Assist in the completion of assessments and other paperwork as assigned by the Program Coordinator or other management staff. Will access HCSIS to complete Incident Reports, ISP documents, and Healthcare Records for 118 Nahant St and as needed for the other individuals
  • Will be On-call for the In-Home Supports programs in absence of the Program Coordinator
  • Will serve as the Human Rights and Fire Safety Officer for In-Home Supports and conduct trainings for staff and individuals on an annual basis
  • Will conduct host Our Health Matters, Craft Night, and similar groups to promote socialization.
Specific Duties for 118 Nahant Street
  • Ensure program files are maintained and updated as needed. for 118 Nahant St.
  • Will work directly in conjunction with the families and service coordinators for the individuals at 118 Nahant St
  • Monitor and facilitate progress of ISP goals for all individuals including 118 Nahant St.
  • Responsible for attending all medical and counseling appointments for the individuals at 118 Nahant.
  • Assist individuals at 118 Nahant St. in developing maximum independence (budgeting, time management, use of leisure time, laundry, transportation, etc.)
  • Oversee the health, nutrition, social and general welfare of program participants for 118 Nahan St
  • Completion of weekly mentor reports for designated individuals at 118 Nahant St.
Qualifications
  • Bachelor’s Degree in Education or Human Services preferred or 3 years’ experience supporting individuals with developmental disabilities.
  • Experience working with families of individuals and professional service providers.
  • Current Medication Administration Certification required.
  • Current CPR and First Aid Certifications required (Will train)
  • Must write and speak fluent English. 
  • A passion and dedication to supporting our people is a must
  • Valid Driver’s license, reliable transportation, and acceptable driving record
  • Must pass company background and reference checks
