Description:

Under the direct oversight of the Program Supervisor and the overall guidance of the Program Manager, engages in highly advanced (senior-level) social services and case management activities.

Delivers Case Management for programs such as Children and Pregnant Women (CPS), Newborn Screening (NBS), Childhood Lead Poisoning Prevention Program (CLPP), Personal Care Services (PCS), Children's Health Insurance Program (CHIP), Child Protective Services (CPS), and case management for families within the Children with Special Health Care Needs Program (CSHCN-SP).

Aids families in navigating the eligibility determination process for CSHCN-SP. Conducts home visits as necessary. Completes intakes, Family Needs Assessments, Service Plans, and Follow-up Services for families on the caseload. Connects families with community resources. Performs comprehensive assessments to ascertain eligibility and necessity for PCS and Community First Choice (CFC). Communicates program services and updates to consumers/public in a clear and positive manner. Actively participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, and community organizations. Provides information and referral services. Completes administrative responsibilities, special assignments, monthly reports, and work activities in a professional and timely manner. Operates under minimal supervision, exercising considerable initiative and independent judgment. Collaborates with public health staff during regional or state emergencies. Upholds confidentiality.

Description:

Maintains case files and provides specialized social work consultation and assessment to families of children who are medically fragile and other identified populations to determine eligibility for Personal Care Services.

Conducts Needs Assessments on all active Case Management cases and completes Individual Service Plans (ISP) with follow-up assessments. Performs follow-up visits with families to evaluate if services have been delivered or are still required. May attend school meetings or group conferences for clients receiving case management. Accurately documents all relevant information in a timely manner. Maintains case files and enters family encounter data into an online database.

(20%) Community & Outreach

Description:
Collaborates with community organizations, other regions, public health entities, and regional program staff. Participates in community events related to case management needs. Provides presentations and information to stakeholders and community members as required.

Engages with local, state, and federal social service entities and others to enhance efforts to support families with children facing health risks, conditions, and special health care needs.

Attends training sessions and seminars to expand knowledge about community resources. Delivers presentations and information to stakeholders and community members as necessary.

Description:

Responsible for documenting all SH&SS activities to achieve regional program goals and objectives, reporting to the Program Supervisor and/or Case Manager Coordinator.

Ensures that all case-related and community activities are accurately recorded in the designated database as specified by central office or regional requirements.

Maintains and documents all work events within the established time frames.

(15%) Quality Assurance

Description:
Develops corrective action plans following the results of Quality Assurance audits. Maintains proper documentation of any QA audits. Records all corrective actions in the designated system. Assists in training new staff, volunteers, and social work interns. Participates in peer reviews for regional quality assurance.

(5%) Adherence to Policy & Other Duties as Assigned

Description:

Employee actively participates and/or serves in a supporting role to fulfill the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

Performs other duties as assigned, including programmatic processes for upcoming events and conferences. Completes program tasks as necessary. Maintains an acceptable driving record, a valid class C Texas driver's license, and professional license. Keeps informed about Human Resources, Information Technology, Administrative Policy, and Program Policy. Reports time, labor, and travel information promptly.
