Hospice Care Nurse Specialist

We are seeking a compassionate and skilled Registered Nurse to join our hospice care team as a Hospice Care Nurse Specialist. As a member of our interdisciplinary team, you will provide high-quality patient-centered care to individuals facing life-limiting illnesses.
About the Role:
- Provide comprehensive nursing care to patients in our inpatient facilities, focusing on symptom management, pain control, and emotional support.
- Collaborate with other healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient.
- Maintain accurate and detailed records of patient interactions, treatments, and outcomes using our electronic medical record system.
- Communicate effectively with patients, families, and caregivers to ensure seamless transitions between care settings.
- Participate in regular team meetings to discuss patient progress, share knowledge, and address challenges.
Requirements:
- Degree in Nursing (BSN or ADN) from an accredited program.
- Current licensure as a Registered Nurse in the State of Georgia.
- Minimum 1 year of experience in hospice or palliative care.
- Ability to lift up to 50 pounds, stand for prolonged periods, and move quickly in response to patient needs.
Salary Range:$65,000 - $85,000 per annum, depending on experience. This role includes a benefits package, paid time off, and opportunities for professional growth and development.
