Job Description:

In this critically important role, you will serve as the primary nurse and case manager for patients receiving care from our in-patient unit. Your expertise will ensure that each patient receives the highest quality services by our interdisciplinary team.

Responsibilities:
  • Respond promptly to calls from patients or family members, providing support and guidance in a professional manner.
  • Perform nursing duties in accordance with Hospice Nursing Standard of Practice and our organization's policies and procedures.
  • Critically evaluate situations and employ problem-solving skills to manage patient and family issues.
  • Maintain exceptional direct care nursing skills, ensuring patient safety and comfort.
  • Document patient care accurately and thoroughly, using our electronic medical records system.
  • Develop and update personalized care plans based on patient needs and our organizational policies.
  • Foster effective working relationships with all staff members to ensure seamless delivery of nursing services.
  • Ensure a safe environment for patients within our facilities.
  • Identify and address emotional and physical pain, adhering to NHPCA guidelines.
  • Supervise licensed practical nurses (LPNs) and certified nursing assistants (CNAs).
  • Accurately receive and transcribe physician orders.
  • Administer medications as prescribed, maintaining accurate narcotic records.
  • Dispose of drugs and narcotics according to established procedure.
  • Triage calls from home patients and notify the on-call nurse as needed.
  • Admit, transfer, and discharge patients as necessary, completing required documentation.
  • Provide emotional support to patients and their families throughout the end-of-life process.
  • Pronounce patients at the time of death, respecting their dignity.
Qualifications:
  • Holds an active RN license in the State of Georgia.
  • Current CPR certification and valid driver's license.
  • Hospice and Palliative Nursing Certification is highly preferred.
Estimated Salary:

$32 per hour, including a comprehensive benefits package and 23 days of paid time off annually.
