Job Summary:

We are seeking a skilled and empathetic Registered Nurse to join our end-of-life care team as an End-of-Life Care Professional. In this role, you will provide personalized care to patients and their loved ones, addressing physical, emotional, and spiritual needs.

About the Role:

  • Assess patients' physical and emotional needs, developing and implementing tailored care plans to meet their unique requirements.
  • Collaborate with other healthcare professionals to ensure seamless transitions between care settings.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and optimal care outcomes.
  • Participate in regular team meetings to discuss patient progress, share knowledge, and address challenges.
  • Maintain accurate records of patient interactions, treatments, and outcomes using our electronic medical record system.

Requirements:

  • Degree in Nursing (BSN or ADN) from an accredited program.
  • Current licensure as a Registered Nurse in the State of Georgia.
  • Minimum 1 year of experience in hospice or palliative care.
  • Ability to lift up to 50 pounds, stand for prolonged periods, and move quickly in response to patient needs.

Salary Range:$60,000 - $80,000 per annum, depending on experience. This role includes a benefits package, paid time off, and opportunities for professional growth and development.
