About Us

CenterWell Primary Care specializes in providing personalized, patient-centered care to individuals managing chronic conditions or recovering from illness, injury, or surgery. Our experienced care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers working together to help patients regain their independence and live healthier lives.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer ongoing care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
  • Confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment.
Requirements
  • Diploma, Associate or Bachelor Degree in Nursing.
  • Minimum of one year nursing experience preferred.
  • Strong med surg, ICU, ER, acute experience.
  • Home Health experience a plus.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
