Job Title: Patient Navigator

Job Overview:
The Patient Navigator plays a crucial role in facilitating patient care within the multi-disciplinary clinic. This position is responsible for assisting with daily operations, outreach initiatives, and connecting patients with essential community resources. The ideal candidate will provide patient education and counseling, ensuring a seamless experience for individuals navigating their healthcare journey.

Key Responsibilities:
1. Provide comprehensive patient counseling and make appropriate referrals based on individual needs.
2. Collaborate with healthcare teams to coordinate patient care effectively.
3. Manage ongoing patient continuity of care throughout their journey in the clinic.
4. Organize and participate in community outreach programs to promote health services.
5. Maintain a high standard of customer service to enhance patient satisfaction within the clinical environment.
6. Develop and administer surveys to assess patient satisfaction and program effectiveness.
7. Engage with hospital and community stakeholders to strengthen patient relations.
8. Conduct follow-ups with referring providers to ensure quality service delivery.
9. Utilize databases to track patient progress and outcomes in outpatient settings.
10. Analyze program data regularly to identify areas for improvement.
11. Maintain accurate documentation and records for all patient interactions and program activities.

Qualifications:
- Required Education: Bachelor’s Degree in Health Education, Social Work, Psychology, Nursing, or a related field.
- Experience: Minimum of 12 months in a medical or clinical care environment. Equivalent experience may be considered in lieu of formal education.
- Certification: Valid license from the State of Oklahoma in a relevant health discipline.

Physical and Environmental Requirements:
- Ability to sit for extended periods and engage in effective communication.
- Work primarily in an office environment.

Preferred Qualifications:
- Bilingual proficiency in English and Spanish is highly desirable.
