We are seeking a highly skilled Registered Nurse Case Manager to join our team at Trinity Health Corporation. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions from acute care to post-acute settings.

Key Responsibilities
  • Conduct thorough assessments of patient goals, needs, and discharge plans
  • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams
  • Coordinate care and services across the continuum of care, ensuring timely and effective transitions
  • Collaborate with physicians and care team members to ensure continued need for acute care hospitalization and explore post-acute care options
  • Ensure accurate and complete documentation of patient information and care plans
Requirements
  • Current Michigan Nursing License
  • Graduation from accredited nursing program (BSN required)
  • Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager
  • Certification in Case Management (CMSA) desirable
