POSITION PURPOSE


The Registered Nurse Case Manager is responsible for overseeing the discharge plans of patients, taking a pivotal role in achieving optimal clinical, financial, and resource utilization outcomes.


This role involves the coordination of care and services for specific patient populations throughout their acute care journey and beyond.


Orientation for this position will last for 6-8 weeks during standard business hours.

KEY RESPONSIBILITIES


The RN Case Manager will conduct thorough assessments of patient objectives, including biophysical, psychosocial, environmental, and financial needs related to discharge planning.


Additionally, the RN will continuously evaluate the appropriateness and timeliness of care levels, diagnostic tests, clinical procedures, and documentation accuracy.


Collaboration with patients, families, and the interdisciplinary team is essential in developing and documenting effective discharge plans.

It is crucial to ensure that all activities related to the discharge plan are executed and adjusted as necessary to meet the evolving needs of patients.


This position requires daily discussions with physicians and care team members to confirm the necessity of continued acute care hospitalization while identifying suitable post-acute care options that align with patient goals.


MINIMUM QUALIFICATIONS
Current Michigan Nursing License

Educational Background:
BSN from an accredited nursing program is required.

Experience:

Minimum of three (3) years of recent direct patient care experience as an RN, demonstrating clinical expertise or equivalent experience in case management.


Certification:
Case Management certification is preferred.


Case managers within the health system are expected to obtain CMSA certification within three years of employment in this role or be actively pursuing certification.
