Position Overview:
ROLE PURPOSE
The Registered Nurse Case Manager is responsible for overseeing the discharge plans of patients, taking a leadership role in achieving optimal clinical, financial, and resource utilization outcomes.
This role involves coordinating care and services for specific patient populations throughout their acute care journey and beyond.
This position is contingent: Please note - Orientation for this role will span 6-8 weeks from 8 a.m. to 5 p.m., Monday through Friday.
KEY RESPONSIBILITIES
The RN Case Manager will conduct thorough assessments of patient objectives, as well as biophysical, psychosocial, environmental, economic, and discharge planning requirements.
Additionally, the RN will continuously evaluate the appropriateness, intensity, and timeliness of care levels, diagnostic tests, clinical procedures, quality, clinical risk factors, and the completeness of documentation.
The RN Case Manager will create and document a comprehensive discharge plan in collaboration with the patient, their family, and the interdisciplinary team.
They will ensure that all activities necessary to facilitate and coordinate the plan are executed and that the plan is adjusted as required based on the evolving needs of the patients.
This role necessitates daily discussions regarding the care plan with physicians and team members to confirm the ongoing need for acute care hospitalization while exploring the most suitable post-acute care settings to align with the patients' needs and goals.
MINIMUM QUALIFICATIONS
Current Michigan Nursing License
Education:
BSN from an accredited nursing program is required
Experience:
A minimum of three (3) years of recent direct patient care experience as an RN, demonstrating clinical expertise or equivalent experience as a Case Manager.
Certification:
Case Management certification is preferred.
Individuals serving as case managers for the health system are expected to obtain CMSA certification within three years of their employment in this role or be actively pursuing certification as a case manager.
