Job Summary

As a Registered Nurse - Case Manager at Trinity Health Michigan, you will play a vital role in ensuring the seamless transition of patients from acute care to post-acute settings. Your expertise in clinical assessment, care coordination, and discharge planning will make a significant impact on patient outcomes and satisfaction.

Key Responsibilities
  • Conduct comprehensive assessments of patients' clinical, psychosocial, and environmental needs.
  • Develop and implement individualized discharge plans in collaboration with patients, families, and interdisciplinary teams.
  • Coordinate care and services across the continuum of care, ensuring timely and effective transitions.
  • Collaborate with physicians and care team members to ensure the continued need for acute care hospitalization and explore post-acute care options.

Requirements
  • Current Michigan Nursing License
  • BSN degree from an accredited nursing program
  • Three years of recent experience in direct patient care as an RN, with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager
  • Certification in Case Management is desirable
