Position Overview:
The primary goal of this role is to enhance the patient journey and clinical results within the Endocrinology Service by facilitating access to care, ensuring seamless transitions between inpatient and outpatient settings, and delivering effective management of chronic endocrine disorders. Middleton Family Medicine is committed to a 'medical home' approach in our specialty services, which includes advanced access methods such as telephone consultations, electronic medical records, and electronic referrals. We understand the importance of long-term management for patients with chronic conditions. In the outpatient environment, we strive for exceptional endocrine management in acute care, necessitating multidisciplinary collaboration.

Key Responsibilities:
The responsibilities of this position encompass, but are not limited to:
  • Providing telephonic management for Endocrinology patients addressing minor concerns based on established protocols.
  • Reviewing and triaging electronic consultation requests, including offering guidance to referring physicians and determining which patients require evaluation in the Endocrinology department.
  • Coordinating care for patients referred to the Endocrinology service.
  • Assisting in the scheduling of Endocrinology hormonal/metabolic testing, biopsies, and procedures.
  • Conducting pre-procedure or testing evaluations, including obtaining medical histories and informed consent from patients.
  • Performing outpatient procedures, such as ACTH stimulation testing.
Required Skills and Competencies:

  • Strong core clinical skills with a willingness and capacity for ongoing learning.

  • Dedication to continuous improvement in clinical practices.
  • Proficiency with electronic medical records.
  • Ability to learn and interpret Continuous Glucose Monitoring data and assist with insulin pump management.
Compensation:$30/hr - $40/hr
Minimum Qualifications:
  • Active Registered Nurse license.
  • Certified Diabetes Care and Education Specialist (CDCES) preferred.
