Position Overview:
The primary objective of this role is to enhance the patient journey and clinical results within the Endocrinology Service. This is achieved by facilitating access to care, ensuring seamless transitions between inpatient and outpatient services, and delivering effective management of chronic endocrine disorders. Middleton Family Medicine is committed to a 'medical home' model in our specialty practices, which includes advanced access options such as telephone consultations, electronic medical records, and virtual visits. We understand the importance of long-term management for patients with chronic conditions. In the outpatient environment, we strive for exceptional endocrine management in acute care settings, necessitating multidisciplinary collaboration.

Key Responsibilities:
The responsibilities of this position encompass, but are not limited to:
  • Managing phone consultations for Endocrinology patients with minor issues based on established protocols.
  • Reviewing and triaging electronic consultation requests, providing guidance to referring physicians, and determining which patients require evaluation in the Endocrinology department.
  • Coordinating care for patients referred to the Endocrinology service.
  • Assisting in the scheduling of hormonal/metabolic testing, biopsies, and procedures related to Endocrinology.
  • Conducting pre-procedure evaluations, including patient history and physical assessments, and obtaining informed consent for those directly referred.
  • Performing outpatient procedures, including but not limited to ACTH stimulation testing.

  • A focus on continuous improvement.
  • Proficiency in using electronic medical records.
  • The ability to learn and interpret Continuous Glucose Monitoring data and assist with insulin pump therapy.
Compensation:$30/hr - $40/hr
Minimum Qualifications:
  • Active Registered Nurse license.
  • Certified Diabetes Care and Education Specialist (CDCES) preferred.
