MDS Coordinator/Restorative Care Nurse

SUMMARY:

The MDS Coordinator is tasked with the precise and prompt completion of all Medicare/Medicaid case-mix documentation to ensure appropriate reimbursement for the care and services rendered within the facility.

This role integrates nursing, dietary, social recreation, restorative, rehabilitation, and physician services to guarantee proper assessment and reimbursement.

Additionally, the incumbent is responsible for the formulation, execution, monitoring, and oversight of the restorative nursing program for the facility, serving as the intermediary between formalized therapy and nursing.

They advocate for a restorative nursing philosophy across facility departments.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Evaluates and determines the health status and care level of all new admissions.
Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
Communicates the care level for new residents to all relevant disciplines.

Coordinates interdisciplinary involvement in completing the Minimum Data Set (MDS) for each new admission to the facility in accordance with regulatory timelines.

Ensures completeness and thoroughness of documentation as mandated by federal, state, and medical standards.
Maintains an accurate schedule of all MDS assessments, including the proper reference dates throughout the residents' stay.
Responsible for the data entry function to ensure accurate data entry and electronic submission of MDS assessments.

Assists and participates in the development, organization, and implementation of the philosophy, objectives, policies, procedures, and standards of nursing care in alignment with the goals of the Care Center Facility and the individual center.

Consults with the Director of Nursing/Assistant Director of Nursing in evaluating the quality of nursing care provided and identifies the need for improvements or changes in nursing practices.

Makes daily rounds to assess patient care, monitor the progress of individual employees, oversee nursing practices, and assist Human Resources with nursing and educational needs.

Maintains awareness of legal aspects of nursing practice, as well as government regulations; demonstrates this in teaching, supervision, and evaluation of patient care.

Conducts rehabilitation assessments for all new admissions and for existing patients throughout the center on a quarterly basis and as needed.

Performs or supervises functional evaluations upon admission, quarterly, and/or as clinically indicated. Collaborates with nurses to make recommendations for restorative care plans. Works with nursing staff to develop restorative care plans.

Ensures that all restorative nursing measures are documented in the care plan as an approach to the focus or need being addressed.

Collaborates closely with the therapy department to ensure that specific recommendations made upon discharge from therapy are implemented.
Reviews weekly documentation completed for residents receiving restorative services. Ensures that all appropriate ADL documentation has been completed for patients.
Oversees incontinence management programs.
Participates in care planning conferences and provides recommendations. Reports on the progress of restorative nursing measures.
Attends Interdisciplinary/QAPI meetings as necessary to provide feedback and identify residents for evaluation of restorative service needs.

Coordinates with Human Resources and the Director of Nursing to orient new staff regarding roles, responsibilities, principles, and practices of restorative nursing.

Works with the Director of Nursing and Therapy to provide ongoing education and evaluation of direct care staff concerning roles, responsibilities, principles, and practices of restorative nursing.

Actively engages in physical restraint reduction and fall management initiatives.

Establishes documentation flow sheets for daily recording of restorative nursing measures for each resident in the program in coordination with medical records.

Performs or supervises the activities of restorative nursing staff.
Acts as a liaison with all therapists: physical, occupational, and speech.
Participates in patient care planning and CMI meetings.
Serves in the capacity of Medicare Services Coordinator (MDS Coordinator) as required.
Prioritizes patient needs; ensures that patients and families receive the highest quality of service in a caring and compassionate environment that recognizes individual needs and rights.
Other duties as assigned.

QUALIFICATIONS:
To perform this job successfully, an individual must be able to execute each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

Registered Nurse with current unencumbered state licensure.
Long Term Care Experience preferred.
Ability to read, write, speak, and understand the English language.
