About the Role

We are seeking a highly skilled MDS Coordinator to join our team at August Healthcare at Wilmington. As a key member of our clinical operations team, you will play a critical role in ensuring the accuracy and completeness of Minimum Data Sets (MDS) and associated processes.

Key Responsibilities
  • Coordinate the completion of MDS and associated processes to ensure accurate and timely submission of case mix data.
  • Chair care plan meetings and ensure that all necessary documentation is completed and up-to-date.
  • Responsible for ensuring the accuracy of individual and facility case mix indices.
  • Accountable for the coordination of documentation supporting the RAI and PPS process.
  • Schedule and coordinate the completion of RAI forms and ensure that assignments are given to appropriate interdisciplinary staff.
  • Monitor 24-hour condition reports for potential significant changes in resident condition.
  • Ensure that MDS data is supported with documentation on the clinical record and accurately reflects the resident's current status.
  • Conduct weekly Medicare meetings and review residents' continuing needs for SNF services and acuity level.
  • Report incidents, accidents, or changes in condition to the RN Supervisor immediately.
  • Maintain the confidentiality of all resident care information, including protected health information in accordance with HIPAA and other federal and state regulations.
Requirements
  • 3 years previous experience in a nursing home with a strong focus in rehabilitative service programs preferred.
  • Knowledge of clinical practices and procedures as well as laws, regulations, and guidelines that pertain to long-term care.
  • RN/LPN with current active licensure in the practicing State.
