Hospice Care Coordinator

We are a reputable hospice agency in Santa Clarita, CA seeking a dedicated and compassionate Hospice Administrative Assistant. This role requires excellent organizational skills, strong communication abilities, and a passion for delivering exceptional patient care.
Job Summary:
This position involves coordinating the patient's and families' care and services, including those of the team, social worker, home health aide, and physician. The successful candidate will be responsible for scheduling appointments, responding to inquiries, and monitoring the re-certification/re-authorization process.
Responsibilities:
- Schedule is Monday-Friday 8:30am-5pm.
- Coordinates the patient's and families' care and services.
- Responds necessary information to the Director of Nursing.
- Monitors the re-certification/re-authorization process.
- Ensures and documents appropriate use of medications, durable medical equipment, and supplies.
- Educates patients, families, and other caregivers.
- Appropriately utilizes the skills and resources of contract personnel.
- Provides care plans and integrates care plans as needed.
- Participates in routine chart reviews.
- Attends regularly scheduled in-services and educational conferences.
Requirements:
- Must be LVN/RN or have equivalent experience.
- Must have a minimum of 1-year Hospice or Home Health Care experience.
- Must possess typing and other clerical skills.
- Fluent in English/Spanish speaking and writing.

Estimated Salary Range:$37,000-$75,000 per year (dependent on qualifications).
