Job Title: Hospice Licensed Practical Nurse

Responsibilities
  1. Implement changes to the plan of care as needed with interdisciplinary collaboration with the support of the hospice medical director and/or the attending physician.
  2. Document accurate assessment of patient status and update the plan of care utilizing appropriate communication.
  3. Provide bereavement resources to the family as appropriate.

Qualifications and Skills
  • Valid LPN License to practice in the state of Pennsylvania.
  • Current BLS/CPR certification.
  • Valid Pennsylvania Driver's license.
  • Strong organizational and communication skills.
  • At least 1 year of experience in Hospice care.

Benefits and Compensation
