As a hospice-licensed practical nurse (LPN) at Anova, you will be a critical part of our clinical care team. With your skill and expertise, you will assist patients in crisis and help with symptom management, wound care, education, and other critical needs.

  • Main Responsibilities:
  • Implementing changes to the plan of care as needed with interdisciplinary collaboration with the support of the hospice medical director and/or the attending physician.
  • Documenting and communicating all calls, interventions, care plan changes, physician contacts within the care coordination notes of the agency's EMR in a manner consistent with policy and procedure.
  • Documenting accurate assessment of patient status and updating the plan of care utilizing appropriate communication (verbal, electronic, written, email documents and databases).
  • Assisting patients, family members or caregivers with concern and empathy; respect confidentiality and privacy and communicate in a courteous and respectful manner.

Qualifications and Skills:

We are seeking an experienced LPN with a valid license to practice in Pennsylvania. You should have at least one year of experience in hospice care and strong organizational and communication skills. Previous wound care experience is preferred.
