Job Title: Hospice Licensed Practical Nurse

Key Responsibilities
  • Implement changes to the plan of care as needed with interdisciplinary collaboration with the support of the hospice medical director and/or the attending physician.
  • Document all calls, interventions, care plan changes, physician contacts within the care coordination notes of the agency's EMR in a manner consistent with policy and procedure.
  • Assist patients, family members or caregivers with concern and empathy; respect confidentiality and privacy and communicate in a courteous and respectful manner.
  • Coordinate services with contracted vendors, outside agencies and interdisciplinary team as needed.

Requirements
  • Valid LPN License to practice in the state of Pennsylvania.
  • Current BLS/CPR certification.
  • Valid Pennsylvania Driver's license.
  • Reliable transportation.

Compensation and Benefits

We offer a competitive salary range of $65,000 - $80,000 per year, based on experience, as well as a comprehensive benefits package, including 401(k) Plan with Company Matching, Medical Insurance, Dental and Vision, Paid Time Off/Vacation, Biweekly pay, Fun Company Events, Employee Discounts, and Referral program.
