Position Overview

The Rehabilitation Specialist plays a crucial role in supporting the Occupational Therapist (OT) at EveryAge by assisting in the collection of objective data for assessments as required. This position is responsible for implementing tailored treatment plans developed by the OT and collaborating closely to establish measurable functional outcomes.

Key Responsibilities
  • Execute occupational therapy interventions that align with established goals, adjusting activities as necessary to promote participant success.
  • Monitor and communicate relevant information regarding participants to the OT, ensuring comprehensive care.
  • Provide consistent updates to the OT and Center Manager regarding participant progress, aiding in the refinement of care plans and discharge strategies.
  • Actively engage as a member of the interdisciplinary team, sharing insights on participant therapy status and notifying the team of any discharge plans as directed by the OT.
  • Assist in the documentation process as instructed by the OT, ensuring accurate record-keeping.
  • Contribute to the development of specialized programs tailored to participant needs and facilitate group activities accordingly.
  • Recommend and oversee the procurement of durable medical equipment (DME) necessary for participants, reporting on DME distribution to the OT.
  • Perform minor repairs on therapy equipment as needed and provide training to staff and caregivers on follow-through programs for participants.
  • Offer guidance to Aides in service delivery and report pertinent issues to the supervisor.
  • Collaborate with the Recreational Therapy Department to adapt activities as required.

This role is essential in enhancing the quality of care provided to participants at EveryAge, ensuring that their rehabilitation needs are met with professionalism and compassion.
