The Licensed Rehabilitation Specialist (PT, DPT, RPT) position in Home Health involves evaluating and assessing patients' care needs related to their physical health, mobility, endurance, pain levels, and other medical issues. After this evaluation, the Rehabilitation Specialist collaborates to create a treatment plan, implements strategies to enhance the patient's health, and tracks their progress.

  • Evaluate the patient's rehabilitation needs and functional capabilities. Assist in formulating the treatment plan with the healthcare provider.
  • Monitor and report the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
  • Administer therapies to alleviate pain, improve and restore function, and/or maintain optimal performance while adhering to professional standards.
  • Educate and inform team members, patients, and family/caregivers about the care plan, functional limitations, and progress towards goals.

License Requirements:

  • Active Rehabilitation Therapy license in the state
  • Valid CPR certification
  • Possession of a current driver's license, vehicle insurance, and access to a reliable vehicle

Additional State Requirements:

  • No additional specific state requirements for AR.
