The Licensed Physical Rehabilitation Specialist (PT, DPT, RPT) in Home Health is accountable for assessing and evaluating patient care requirements related to mobility, endurance, pain, and other health conditions. Based on this assessment, the Rehabilitation Specialist collaborates to formulate a treatment strategy, implements interventions aimed at enhancing the patient's health, and monitors the patient's progress.
- Assesses the patient's functional capabilities and physical therapy requirements. Aids the physician in crafting the therapy care plan.
- Observes and communicates the patient's reactions to treatment and any alterations in their condition to the supervising nurse and/or physician.
- Provides treatment to alleviate pain, restore function, and/or maintain optimal functioning in accordance with established standards of practice.
- Educates and informs relevant members of the healthcare team, the patient, and their family/caregivers regarding the care plan, functional limitations, and progress towards objectives.
Licensure Requirements
- Current Physical Therapy licensure in the state
- Current CPR certification
- Must possess a valid driver's license and vehicle insurance, along with access to reliable transportation
