Key Responsibilities- Client Support and Services
- Develop and implement individualized treatment plans to meet the unique needs of assigned clients.
- Provide case management, service coordination, and supportive counseling to promote client independence and well-being.
- Assist clients with daily living skills, shopping, budgeting, and housing needs.
- Facilitate access to income support, entitlements, and mental and physical health services.
- Client Identification and Outreach
- Identify and engage with community members in need of support services.
- Develop and implement outreach strategies to promote service awareness and accessibility.
- Advocate for clients' rights and interests, ensuring their voices are heard and respected.
- Information Management and Record Keeping
- Maintain accurate and up-to-date clinical and administrative records.
- Ensure timely completion of case management documentation and billing requirements.
- Protect client confidentiality and maintain secure record-keeping practices.
- Communication and Collaboration
- Establish and maintain effective communication with clients, families, and service providers.
- Collaborate with internal and external stakeholders to ensure seamless service delivery and coordination.
- Preferred Bachelor's degree in Social Work, Psychology, or related Human Services field.
- Equivalent experience of 1-3 years will be considered.
- Valid Driver's License.
- Strong typing and computer skills.
