The Health Resource Manager is responsible for providing case management services for SVMC subacute patients. This role requires a strong clinical background and excellent communication skills to coordinate patient-centered plans of care.
Key Responsibilities:
• Conduct intake interviews to identify holistic plans of care
• Facilitate multiple aspects of care coordination with patients, families, providers, and community resources
• Develop patient transition plans and provide education to patients and caregivers
• Advocate for patient needs and goals across the interdisciplinary team
• Collaborate with providers to improve quality and efficiency of care
Requirements:
• Graduation from an accredited school of nursing and a Bachelor's of Science in Nursing preferred
• Valid RN license and current matriculation into a BSN program may be considered
• Prior experience in utilization review, discharge planning, and/or case management preferred
• 3-5 years of recent clinical experience required
• VTNursing License required by date of employment
• Case Management Certification required within 2 years of employment
