Job Summary

We are seeking a highly motivated and compassionate Outreach Worker to join our team at Operation SafeHouse. As a key member of our outreach team, you will provide critical support services to homeless youth, empowering them to transition into stable living environments.

Key Responsibilities
  • Conduct street-based crisis intervention services, gateway services, referrals, and crisis support counseling to homeless youth on the streets.
  • Work closely with all street youth who voluntarily enter into SafeHouse programs, providing individualized support and guidance.
  • Develop and implement comprehensive case management plans, ensuring seamless transitions for youth leaving the streets.
  • Provide continuous transportation and support throughout the case management process.
  • Enroll clients into Medical and CAL Fresh programs, promoting access to essential services.
  • Assist clients in becoming document-ready, including IDs, birth certificates, and social security cards.
  • Develop and implement individualized 'Wellness Plans' that address housing, services, and wellness needs.
  • Connect clients to medical and mental health resources as needed, ensuring comprehensive support.
  • Complete daily time and activity sheets, tracking progress and outcomes.
  • Complete all intake paperwork for youth participating in SafeHouse outreach services.
  • Track the progress of youth who have entered housing programs, following up for up to 3 months post-program.
  • Conduct street outreach with other team members, prioritizing personal safety and community engagement.
  • Submit monthly reports to the Outreach Director, maintaining statistical data as assigned.
  • Maintain a clean driving record, keep company vehicles clean, and report maintenance issues to the agency.
  • Attend community fairs to promote awareness of agency programs and engage with street youth, building trust in our services.
  • Maintain professional boundaries with clients, ensuring appropriate relationships.
  • Complete applicable paperwork, including Apricot and VISPDAT, and enter data into HMIS.
Requirements
  • High School Diploma or equivalent experience.
  • Experience working with at-risk youth or homeless populations.
  • Excellent listening and problem-solving skills.
  • Ability to work as a team and build strong community partnerships.
  • Excellent oral and written communication skills; computer and word processing skills.
  • Current driver's license, clean driving record, and reliable transportation.
  • Pass state criminal background check.
Work Environment
  • Work indoors and outdoors.
  • Fast-paced environment.
  • Moderate to loud noise level.
  • Driving to site locations.
Physical Demands
  • Must be able to remain in a stationary position.
  • Light work, including moving objects up to 20 pounds.
  • Occasional movement within the office to access file cabinets, office machinery, etc.
  • Constantly works in outdoor weather conditions.
  • Move from one location to another to provide program services to youth on the streets.
