Job Summary
We are seeking a skilled Patient Care Coordinator to join our team at UofL Health - Frazier Rehabilitation Institute. The successful candidate will be responsible for providing exceptional patient care and support services to our patients and families.
Key Responsibilities
- Anticipate and accommodate patient needs by responding to their verbal and nonverbal cues.
- Prepare patients for daily therapy sessions and provide opportunities for independence and decision-making.
- Assist with patient admission, transportation, and discharge, as well as provide support with daily living activities.
- Recognize changes in patient status and report relevant information to registered nurses.
- Ensure patient rights, privacy, and confidentiality are protected.
Requirements
- High school diploma or equivalent required.
- Basic Life Support (BLS) certification accredited by the American Heart Association (AHA) required.
- Experience in a healthcare setting preferred.
- State Registered Nurse Aide (SRNA) certification by the Kentucky Board of Nursing preferred.
