Key Responsibilities
  • Assessing patients' psychosocial needs, developing written plans of care, and following up with patients and documenting in Epic
  • Consulting with patients, families, caregivers, staff, and providers on psychosocial needs and concerns
  • Assisting the care team and facilitating patient-family meetings for goal setting and advance care planning
  • Linking patients to community resources, including financial assistance, transportation, food, housing, and support groups

You will also serve as a resource to physicians and staff on community resources and act as a primary communicator about social service programs available in the community.

About the Requirements

To be considered for this role, you must have a Master's degree in an accredited social work educational program and be a Licensed Social Worker Associate-Advanced (LSWAA), Licensed Advanced Social Worker (LASW), Licensed Social Worker Associate-Independent Clinical (LSWAIC), or Licensed Independent Clinical Social Work (LICSW).

A minimum of 1 year of healthcare experience is required, with preferred qualifications including clinical work with geriatric/palliative care patients and their families.

About the Salary

The estimated annual salary range for this role is $58,300 to $114,300.
