Job Summary:The PTAC Case Manager will play a critical role in managing capacity and optimizing patient flow across our system. This includes coordinating with hospital-based Case Managers, capacity leaders, and other stakeholders to identify opportunities for improvement and implement solutions. The successful candidate will have excellent communication and problem-solving skills, with the ability to work independently and as part of a team.
Key Responsibilities:- Coordinate with hospital-based Case Managers and capacity leaders to identify opportunities for capacity optimization and implement solutions.
- Develop and maintain strong relationships with hospital-based Case Managers, capacity leaders, and other stakeholders to ensure effective communication and collaboration.
- Monitor system-wide capacity and develop strong knowledge of hospital and non-hospital assets throughout MGB, as well as capabilities at non-MGB facilities.
- Identify and manage scenarios in which a patient may benefit from alternative levels of care or alternate placement, including out-of-network transfer requests, out-of-state and transfer requests with anticipated difficult disposition, and home-based services.
- Work closely with colleagues in the Specialty Desks within the MGB PTAC to help address barriers to discharge that are escalated from the MGB hospitals or other assets.
- Participate in MGB huddles representing Case Management and complete other duties as assigned.
- Graduate of an approved school of nursing, BSN required. MSN preferred.
- Current license to practice as a registered nurse in the State of Massachusetts.
- Case Manager Certification preferred.
- 5+ years of Case Management experience at an acute care hospital.
- Must have extensive knowledge of case management and experience designing and/or utilizing innovative approaches to optimizing capacity, inflow/outflow, and saving bed-days.
